Application
- Complete one application per child. For full consideration of admission, this application must be completed in its entirety.
School Records
- Provide copies of the applicant's two most recent report cards & standardized test reports (if applicable)
- Submit the Authorization for Release of School Records form.
Questionnaires
- Middle School Student Questionnaire (to be filled out by 6th - 8th grade applicants)
- TK/K Questionnaire (to be filled out by the parents of TK & K applicants)
Recommendation Forms
- Give recommendation forms to applicant's current teacher (1st - 8th) /school administrator (TK-K).

Additional Notes

Applicants for Transitional Kindergarten must be 4 by June 1st of the academic year they are applying.
Applicants for Kindergarten must be 5 by June 1st of the academic year they are applying.
All applicants will be assessed by an administrator and/or classroom teacher prior to admission, and will complete a grade academic screening process to determine whether St. Mark curriculum fits their educational needs. You will receive information on how to schedule an appointment after your application has been submitted.
Other factors that will influence admission:
- Siblings - relationships
- Parishioners – Church of St. Mark parish involvement and membership
- Involvement and membership in another Catholic parish
- Commitment to Catholic education through the elementary years
Families will be notified of the admission decision and the enrollment procedure either in late February or early March (for TK & K applicants) or within weeks of completion of the application process (all other grades or late applications).
Upon acceptance and prior to enrollment, parents must provide immunization records and a completed Report of Health Examination for School Entry.
