Application Forms
For full consideration of admission, the application and all required documents (outlined below) must be completed in its entirety. Please submit one application per child.
- Application:
- Submit a completed Application
- Applicant Photo
- School Records:
- Provide copies of the applicant's two most recent report cards & standardized test reports (if applicable, 1st - 8th applicants)
- Submit the Authorization for Release of School Records form.
- Questionnaires
- Middle School Student Questionnaire (to be filled out by 6th - 8th grade applicants)
- TK/K Questionnaire (to be filled out by the parents of TK & K applicants)
- Baptismal Certificate (if applicable)
- Birth Certificate
- Confidential Recommendation Forms
- Give recommendation forms to applicant's current teacher (1st - 8th) /school administrator (TK-K).
- Immunization Records
