We're excited that you've chosen to apply for admission at St. Mark School and we can't wait to learn more about your child/ren and family.  Following are all of the documents required for full admission consideration.  Please ensure they are completed in their entirety and submit one application per child. 1.   Application - Submit a completed Application

2.   Recent Photo of Applicant 

3.   School Records:

   • Provide copies of the applicant's two most recent report cards & standardized test reports (if applicable, 1st - 8th applicants)

   • Submit the Authorization for Release of School Records form.

4.   Questionnaires:

   • Middle School Student Questionnaire (to be filled out by 6th - 8th Grade Applicants)

   • TK/K Questionnaire (to be filled out by the parents of TK & K applicants)

5.  Baptismal Certificate (if applicable)

6.  Birth Certificate

7.  Confidential Recommendation Forms

   • Please ask the applicant's current teacher (1st-8th) or school administrator (TK & K) to complete a Recommendation Form.  

8.   Immunization Records

9.   $100.00 Application Fee

   • Payable via check, cash, or Venmo (@saintmarkschool). If paying by Venmo, please note "Application Fee for (Insert Applicant's name)"
